Care for Complex Co-occurring Disorders
Care for Complex Co-occuring Disorders (CCD), is a prospective cohort study that examines individuals with co-occurring psychiatric, substance use disorders and physical health conditions to track trajectories of service use and health outcomes in this population. Traditional models of care treat each condition as independent, resulting in follow-up care that is fragmented and siloed, and a health system that is difficult to navigate. By focusing on clinical characteristics, patterns of health service use and health outcomes, this cohort study is designed to advance knowledge and inform best practices to optimize policy reforms needed for delivery of holistic, person-centered care in this population. Direct impacts of this research include targeted health care reform, improved access to services, and promotion of continuity of care thus helping to ensure individuals with concurrent psychiatric, substance-use and physical health needs are receiving the right care, at the right time in the right place.
